We require that every patient fill out an intake form every year. Medications change, surgeries occur, major changes of health, changes of cannabis use patterns, etc., which all inform the provider (us) so that we can provide the best guidance and support to you, the patient!

 

Please have your driver's license and proof of residency handy when filling out our form, as you will need to upload a photo of each for your registration.
